The blood itself can run too thin to feed the brain.

Your brain runs on oxygen delivery, which is blood flow times the oxygen the blood carries. Anemia cuts the second half: fewer red cells, less hemoglobin, so the brain runs short even with the pipes wide open. It speeds the flow to compensate, until it cannot. You feel the shortfall as fog and fatigue; the more important question is why the count is low in the first place.

Why it reaches the brain

The brain runs on delivery, not just flow.

Anemia cuts the oxygen the blood carries, not the flow, so the brain runs short and speeds its flow to keep up, until it cannot.

What the brain actually needs is oxygen delivery, and that is two things multiplied: how much blood arrives, and how much oxygen that blood carries. Almost all of the oxygen rides on hemoglobin, so anemia, a low hemoglobin, cuts the second half of the equation. The pipes can be wide open and the flow normal, and the brain still comes up short.1 The body answers the only way it can: it speeds the blood up. As hemoglobin falls the brain widens its vessels and the thinner blood flows faster, raising cerebral blood flow to hold oxygen delivery roughly steady, about 18 percent higher after a moderate drop.2 Lower hemoglobin shows up on brain scans as exactly that, higher resting flow.3

The compensation has a floor

Speeding the flow buys time, not unlimited room. A healthy brain can hold oxygen delivery up by this trick down to a hemoglobin of about 5 to 6 grams per deciliter; below that the reserve is spent, delivery finally falls, and thinking measurably slips, though it recovers once the anemia is corrected.1 A brain already strained by injury or disease runs out of room far sooner. And a brain spending its flow reserve just to stay supplied has little left to handle anything else.2

The everyday signs are the brain and body working harder for the same oxygen: fatigue, brain fog, poor concentration, lightheadedness, headache and breathlessness. Push hemoglobin low enough and cognition measurably drops, and it lifts again when the count is restored.1

The long shadow

Over years, and at the extremes, the associations turn serious. In older adults, anemia tracks a roughly 50 percent higher risk of later dementia,4 and anemia on admission is tied to worse survival after a stroke.5 These are associations, and low oxygen delivery to an already-strained brain is a plausible thread. Anemia rarely travels alone, so a low count is worth taking seriously and worth explaining.

Before the count even drops

Iron runs the brain before the blood count falls.

Iron is not only for red cells. The brain needs it to build dopamine and to insulate its wiring, and the body empties its iron stores before it fails to fill red cells, so iron can run low in the brain while the blood count still looks normal. That is why iron deficiency without anemia is a real and treatable cause of fatigue.6 And there is a practical catch worth knowing: a routine blood test measures your hemoglobin, so it will catch anemia, but it often does not include ferritin, the measure of your iron stores, unless you ask for it. A normal blood count can sit right on top of an empty iron tank.

Restless legs

The clearest example is restless legs syndrome, the night-time urge to move that wrecks sleep. It tracks a shortage of iron in the brain, in the very regions that run dopamine, and it can occur even when the iron in your blood looks fine.7 Topping up iron often settles it, which is why a low-normal ferritin is worth checking when sleep is the problem.

Not all the same

Three common ways the blood runs thin.

Iron deficiency is the commonest, the world’s most common nutritional deficiency,8 and it is usually a slow loss: heavy periods, pregnancy, a quiet bleed in the gut, or poor absorption. Stores fall first, so ferritin, the marker of iron in the bank, drops before hemoglobin does.9

The one that hits the nerves

Vitamin B12 deficiency is the one to know on a brain site, because it damages nerves and the spinal cord and can cloud thinking before it ever shows in the blood count. In a classic series, more than a quarter of people with B12 nerve damage had no anemia at all.10 A normal blood count does not rule it out.

When the iron is there but locked away

Anemia of inflammation rides with chronic illness, kidney disease, cancer and long-running inflammation. The body raises a hormone called hepcidin that traps iron inside its own cells, so the iron is present but the marrow cannot reach it.11 Here the fix is treating the disease, not swallowing more iron.

A clue, not a diagnosis

A low count is where the real question starts.

The number is only the start: the cause has to be found, and iron should never be self-prescribed without testing first.

Anemia is a sign. The job is finding why. New iron-deficiency anemia in an adult man, or a woman past menopause, is taken seriously precisely because it can be the first hint of a slow bleed, and sometimes a cancer, in the gut: guidelines send these patients for a camera test of the stomach and bowel, which finds a lower-gut cancer in about one in eleven and an upper-gut one in about one in fifty.9 That is not cause for panic; it is the reason a low count should never just be topped up and forgotten.

Get medical advice promptly if
  • your stools are black and tarry or visibly bloody, a sign of bleeding (iron tablets also blacken stool, so do not assume);
  • the anemia comes with chest pain, marked breathlessness, or fainting; with chest pain or breathlessness, get urgent care;
  • you are anemic and cannot say why, especially as a man or after menopause, where the cause needs to be found, not just covered with iron.

Do not just take iron. Swallowing iron without testing can do harm in people who already store too much of it, and it can hide the very cause a clinician needs to find. Get tested instead, with ferritin the most useful single marker, though it can read falsely normal when inflammation is present.9 This is information, not a diagnosis: let a clinician confirm the anemia and find what is behind it.

What to do

Most anemia is very treatable, once you know the cause.

The common causes answer well to treatment, and cheaply. Iron deficiency usually responds to cheap oral iron, carried on for a few months past a normal count to refill the stores; taking it every other day is often gentler on the gut and better absorbed, though it does not correct an established anemia any faster.12 When tablets are not tolerated or not enough, iron can be given by a drip, and a true B12 deficiency is replaced by injection.9

Treat the source, not just the number

Topping up iron or B12 fixes the blood, but the point is to fix what drained it: the heavy periods, the diet, the bleed, the inflammation. Get that found and handled, and the brain gets its oxygen back.
